Whether you're coming to Schulte Family Chiropractic as a chiropractic patient, a nutrition patient or both, your first visit will be an important step toward optimal health and wellness. At this visit, we'll gather the health information and data we need to create your custom care plan. Many of our patients learn some amazing things about themselves following the initial evaluation. The prospect of finding answers to both chronic and acute pain and health issues as they relate to the spine and/or nutrition brings many of our patients hope and relief.
An Overview of the Visit
When you enter our office, you will be greeted by one of our friendly staff members and escorted to the waiting area to complete a few short forms. Or you can save time by completing the intake forms before you come in:
Many of our patients' insurance plans contribute to their chiropractic care. Please bring your insurance card with you and we will call to verify those benefits.
Once your registration is complete, we will give you a tour of the office and head back to the exam room to get you started. You will watch a brief video and then meet with Dr. Schulte for a one-on-one consultation.
